Comprehensive Community Health Assessment "CHA"

Three Rivers District Health Department serves the four-county regions of Carroll, Gallatin, Owen, and Pendleton Counties in Kentucky.  With the exception of the city of Carrollton, which is identified as an “urban cluster”, having a population of more than 2,500, but less than 50,000, all of the territories served are considered rural.

This comprehensive community health assessment is intended to be used by the community to identify opportunities for improvement and develop goals and plans to implement change to better the health of the populations living, learning, working, and playing in our communities.

The health outcomes, health factors, policies, and programs for the populations therein measure the health of a community. Individual factors such as health behaviors, clinical care, social and economic aspects, and the physical environment directly affect the health outcomes, life expectancy, and quality of life of a community. Life expectancy and quality of life are strong indicators of health and subsequently carry a greater weight in determining the overall health of a community.

The health factors and outcomes we have assessed include social determinants of health (SDOH), or “the conditions in places where people live, learn, work, and play” that affect a wide range of health risks and outcomes.

Demographics and social economic characteristics are those attributes and details specific to the populations in a particular area such as race, ethincity, gender, age, housing, employment, and income. It includes the who, what, when and where of a given population.

Health Risk Factors

Health Risk Factors are attributes and characteristics in a given population that increase the likelihood of a person developing a disease or condition. They include health behaviors, clinical care, social and economic factors, as well as physical environment factors.

Community Themes and Strengths

The Community Themes and Strengths is a survey conducted to assess the view and understanding of health, and issues affecting health in the communities we serve. It addresses such questions as what is important to our community, how is quality of life perceived and what assets do we have that can be used to improve community health?
